Welcome to our office!  We look forward to meeting with you to find out how we can be of help.  You do not require a referral by a dentist or physician to schedule an appointment to meet with Dr. Freilich.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or a treatment plan involving larger surgical procedures, deep sedation or general anesthesia will require a pre-surgical evaluation before your surgery can be scheduled.

If you are taking Coumadin (Warfarin), please advise our office before your appointment.  You will require an INR blood test, arranged by your family doctor, before your surgical procedure can be completed.

Please assist us by providing the following information at the time of your consultation:

  1. If you are being referred by a dentist or physician, please bring your referral note and any x-rays if applicable.
  2. A list of medications you are presently taking.
  3. If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.), or if you require prophylactic antibiotic coverage for dental procedures.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, our surgeons and staff will discuss this with you at your consultation.
